About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Care Team at Hospice Of Cleveland County Inc. As a Hospice Nurse, you will play a vital role in providing end-of-life care to patients and their families.

Key Responsibilities
  • Delivery of Patient Care
    • Coordinate and deliver patient/caregiver care
    • Conduct nursing assessments to manage patient/caregiver needs
    • Develop, implement, evaluate, and update the plan of care to ensure uninterrupted care and achieved goals
    • Facilitate patient and caregiver independence
    • Utilize patient-related protocols, standing orders, and guidelines when providing hospice care
  • Interdisciplinary Group
    • Attend, participate, and facilitate interdisciplinary group conferences and special care conferences
    • Establish and maintain effective working relationships with team members
    • Identify patient/caregiver needs and make referrals to appropriate team members
  • Documentation
    • Ensure accurate, complete, succinct, and timely clinical documentation in accordance with guidelines and licensure and certification standards
  • Professional and Personal Accountability
    • Demonstrate competency in knowledge and skills
    • Ensure VIA Health Partners' professional reputation is maintained and projected
    • Assume responsibility for professional development and staying abreast of current trends in the healthcare field
Requirements
  • Diploma or associate degree in nursing required; Bachelor's Degree preferred
  • Current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) required
  • Current CPR required; CHPN preferred
  • Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions
  • Experience in direct care of patients with life-limiting illnesses required
