About Us:
Central City Concern (CCC) is committed to serving over 13,000 individuals annually who are experiencing or at risk of homelessness. We provide essential services, including affordable housing, health care, addiction recovery, and employment assistance, fostering a pathway to stability and success.
Key Responsibilities:
As an RN Case Manager, you will:
1. Collaborate with a multi-disciplinary team to deliver intensive case management and integrated treatment for dual diagnoses.
2. Conduct thorough mental health assessments and develop personalized treatment plans in partnership with clients.
3. Engage in outreach activities to connect with clients and facilitate their access to necessary services.
4. Monitor client progress and adjust care plans as needed to ensure optimal outcomes.
5. Administer medications and provide direct care in accordance with established protocols.
6. Participate in case conferences and clinical meetings to enhance team collaboration and treatment effectiveness.
Qualifications:
To be successful in this role, candidates must possess:
- A valid RN license in the State of Oregon.
- Certification with MHACBO as a QMHA-R or QMHA I.
- A minimum of two years of experience in psychiatric or addiction nursing.
- Strong communication and interpersonal skills to effectively engage with clients and team members.
Preferred Skills:
Experience as a Qualified Mental Health Professional (QMHP) is advantageous.
