- Education: Bachelor of Science in Pharmacy or Pharm. D. degree.
- Licensure: Active Pharmacy License in the state in which employed.
- Certification: Immunization Certification through an accredited organization (e.g., APhA).
- Other: Valid Driver's License and no pending felony charges or convictions for criminal offenses involving controlled substances.
