Pediatric Private Duty LPN Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Pediatric Private Duty LPN. As a key member of our home care team, you will provide high-quality, patient-centered care to acutely ill and chronically ill pediatric patients in their homes.

Responsibilities:
  • Provide direct nursing care as prescribed by the physician to pediatric patients in their homes.
  • Record clinical and progress notes, and coordinate other patient care activities/services to provide the highest quality of patient care.
  • Work under the direct supervision of the Registered Nurse and/or designated supervisor.
  • Perform nursing activities within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conference, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinator.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.
  • Strong patient management and documentation skills.
  • Ability to work flexible hours as required to meet identified patient needs.
