Job Overview

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time provider. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a diverse range of patients and provide comprehensive care in a variety of settings, including home visits, skilled facilities, and telehealth.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and counseling to patients and their families
  • Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
  • Document patient information and outcomes in a timely and accurate manner
Requirements
  • Masters degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
