Patient Care Coordinator for Acute Care Hospitals

Cayuga Health System is seeking an experienced LMSW Case Manager to join our team in the Inpatient Units. This role is responsible for ensuring the delivery of high-quality care to assigned patients, assessing their needs, and developing effective Plans of Care.
The ideal candidate will have strong communication and organizational skills, as well as experience working in acute care hospital settings. A Master's degree in Social Work, current NY State Licensure (LMSW), and at least 1-3 years' experience in an acute care hospital setting are required.
Key Responsibilities:
- Evaluate hospital admissions using evidence-based standards of care to ensure medical necessity and appropriate level of care.
- Communicate effectively with providers and care teams to capture and document patient status.
- Actively participate in multidisciplinary rounds and issue federal and state notices as needed.
- Serve as discharge planner for patients, assessing risk for readmission and identifying community support systems.
The estimated salary for this position is $41,500-$56,800 per year, depending on experience and qualifications.
