Transition Specialist

Job Summary
The State of Kansas is seeking a skilled Transition Specialist to join our team. As a Transition Specialist, you will play a critical role in supporting students with disabilities as they transition from school to employment, post-secondary education, or the vocational rehabilitation program.
Key Responsibilities
- Conduct and document outreach and education activities in assigned school districts and/or special education co-ops, including outreach to transition personnel, special education teachers, and building administrators.
- Respond timely to referrals and requests for Pre-ETS services for Pre-ETS eligible or potentially eligible students and assess their needs, collect necessary releases of information, and make referrals as appropriate.
- Make referrals to the traditional vocational rehabilitation (VR) program, including development of a referral package to aid in timely determination of eligibility for VR and service needs.
- Provide Pre-ETS related progress reports to VR counselors as needed to ensure smooth and effective VR transition services.
- Document and determine Pre-ETS eligibility, create files, and enter required data in the state agency computer system, KMIS, Pre-ETS module.
- Develop a Pre-ETS services plan with eligible students, their legal representatives, and other support individuals as requested, and implement the plan, including the 5 required services: job exploration counseling, work-based learning experiences, counseling on opportunities for enrollment in comprehensive transition or postsecondary educational programs, workplace readiness training, and instruction in self-advocacy.
- Attend and participate in transition IEP meetings as invited and appropriate.
- Maintain strong professional relationships with school district and co-op transition-related personnel to ensure effective transition for eligible students.
- Participate in job-related training and education to ensure current knowledge of the VR program, special education transition, and other needed skills and knowledge.
Requirements
- Education: General; Five years of experience interviewing, investigating, compiling information, documenting decisions, interpreting guidelines, and/or providing technical assistance relevant to the agency's programs. Post-secondary education may be substituted for experience as determined relevant by the agency.
- Two years of experience working with students and/or adults with disabilities.
- Valid Driver's License (valid license must be maintained throughout employment).
Preferred Qualifications
- Bachelor's degree.
Post-Offer, Pre-Employment Requirements
- Security Clearance: A background check is required for all positions with the Department for Children and Families as a condition of employment, which may include fingerprinting and FBI check. Security clearance must be obtained prior to employment with DCF. Must maintain security clearance throughout employment.
- Tax Clearance: Candidates offered employment in a State of Kansas position will need to apply for a Kansas Tax Clearance Certificate within 10 days of being offered a position.
