Job Summary

We are seeking a skilled Rural Registered Nurse Case Manager to join our team at the Southeast Alaska Regional Health Consortium (SEARHC). As a key member of our nursing department, you will be responsible for coordinating patient care, promoting quality and cost-effective healthcare outcomes, and collaborating with interdisciplinary teams to establish individualized medical goals.

Responsibilities
  • Manage SEARHC patients, coordinating their care and ensuring timely and appropriate treatment
  • Collaborate with medical providers and other team members to develop and implement individualized care plans
  • Coordinate multi-disciplinary care conferences for high-risk or complex patients
  • Review EHR reminders and triggers, providing outreach and education to patients as needed
  • Participate in case management reviews and work closely with multidisciplinary teams to ensure comprehensive care for patients
  • Foster patient and family decision-making, independence, self-care, and growth and development
  • Use problem-solving and anticipatory skills to meet patient needs promptly and efficiently
Specialty Clinics
  • Facilitate coordination of specialty clinic referrals, managing waitlists and reviewing referrals for appropriateness
  • Prepare clinical space for specialty clinics, setting up equipment and assisting providers during procedures
  • Provide clinical support for specialty clinics, including assessment, documentation, education, and medication administration
OB Case Management
  • Serve as primary case manager for OB patients, responsible for intake and tracking processes
  • Provide education and outreach to OB patients, working closely with primary care providers and case managers to ensure comprehensive care
Required Skills and Qualifications
  • Graduate of an accredited school of nursing, Bachelor's degree preferred
  • Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required
  • Direct Hire must have applied for an Alaska nursing license before start date; Agency staff must have an active Alaska license
  • Basic Life Support certification preferred; National Case Management certification preferred
  • At least 1 year of general nursing experience, with at least 6 months' employment as a clinical nurse at a SEARHC outpatient clinic or at least 2 years of general nursing experience in an ambulatory setting or 1 year case management experience
