Oncology Support Services Manager

Patient Navigation Program
The Tidelands Health Cancer Center Network's Patient Navigation Program is designed to improve patient outcomes and reduce barriers to care. As a Patient Navigator, you will play a vital role in coordinating patient appointments, facilitating connections between patients and necessary support services, and maintaining accurate and detailed records of patient interactions and outcomes.
Responsibilities:
- Coordinate patient appointments and ensure timely follow-up care
- Collaborate with providers to develop and implement patient care plans
- Facilitate connections between patients and necessary support services
- Maintain accurate and detailed records of patient interactions and outcomes
- Participate in quality improvement initiatives to enhance patient care and satisfaction
Qualifications:
- An Associates degree in nursing is required, with a BSN preferred
- At least 5 years of hospital nursing experience, outpatient preferred
- At least 2 years of professional experience in an oncology or outpatient infusion setting required
- Previous chemotherapy experience required
- Previous patient navigation experience is preferred
- Previous experience working with culturally diverse populations is required
- A strong working knowledge of Oncology Programs is essential
- Strong understanding of community resources for oncology patients and assisting with referrals
- Demonstrated commitment to impacting the care of high-risk patients
- Knowledge of complex patient issues is preferred
- Able to establish strong working relationships within the medical community is required
