About the Role

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Phoenix Home Care and Hospice. As a dedicated nurse, you will provide high-quality care to children and young adults with specialized healthcare needs in the comfort of their own homes.

Responsibilities
  • Deliver individualized nursing care to patients based on their unique needs and treatment plans.
  • Assist in the development, review, and revision of the plan of care.
  • Build strong emotional connections with patients and their families to offer a supportive and reassuring presence.
  • Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician.
  • Provide ongoing instruction and supervision of clients, families, or other health team personnel in the plan of treatment.
  • Monitor and record patient conditions and document provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements
  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multi-task.
  • Excellent Communication Skills.
  • Positive, Professional attitude.
