Home Health Case Manager

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Case Manager in the 5 Towns, Long Beach, Rockaways area.

Salary:
This full-time position offers a competitive salary range of $113,000 to $120,000 annually, plus a $20,000 sign-on bonus. Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.
About the Role:
As a Registered Nurse Case Manager, you will perform patient assessments, collaborate with the care team to develop and implement a plan of care, and promote patient health and independence through teaching and appropriate rehabilitative measures. You will also supervise LPNs and HHAs, complete documentation timely and accurately, and regularly communicate patient progress to the clinical manager and care team.
Responsibilities:
* Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
* Make referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
* Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
* Supervise LPNs and HHAs.
* Complete documentation timely and accurately.
* Regularly communicate patient progress to the clinical manager and care team.
Required Skills and Qualifications:
* One (1+) year of clinical experience as a Registered Nurse (RN).
* Current RN license, specific to the state(s) you are assigned to work.
* Current CPR certification.
* Valid driver's license, reliable transportation and liability insurance.
