Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' physical and emotional needs, develop individualized care plans, and communicate with healthcare teams to ensure seamless care coordination.
  • Medication Management: Ensure patients receive prescribed medications and treatment supplies, update files as necessary, and communicate with healthcare teams to address any concerns.
  • Education and Support: Provide education and support to patients and their families on disease management, medication adherence, and healthy living practices.
  • Documentation and Communication: Accurately document patient information, communicate with healthcare teams, and maintain confidentiality in all interactions.
  • Team Collaboration: Work collaboratively with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Quality Improvement: Participate in quality improvement initiatives, identify areas for improvement, and contribute to the development of evidence-based practices.
  • Professional Development: Stay current with industry developments, attend educational conferences, and engage in ongoing professional development to enhance clinical skills and knowledge.
Requirements
  • Education: Associate Degree in Nursing or higher from an accredited nursing program.
  • Experience: Minimum one year of experience as a Registered Nurse in a medical-surgical setting, acute care, or home care environment.
  • Licensure: Current RN licensure in good standing, CPR certification, and TB screening.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and strong problem-solving and critical thinking skills.
Work Environment

As a Home Health Registered Nurse with Residential Home Health and Hospice, you will have the opportunity to work in a dynamic and supportive environment, providing care to patients in their homes. You will be required to drive daily for meetings and visits to residential homes, and may be exposed to changing weather conditions.
