Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their physical, emotional, and cultural needs. Develop and implement individualized plans of care to address these needs and promote optimal patient outcomes.
- Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
- Wound Care and Management: Provide wound care and management services to patients, including dressing changes, wound cleaning, and debridement.
- Patient Education: Educate patients and their families on disease management, medication use, and self-care techniques to promote healthy behaviors and improve patient outcomes.
- Collaboration and Communication: Work effectively with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to coordinate patient care and ensure seamless transitions.
- Education: Graduate of an accredited school of nursing with a current RN license in the state of Connecticut.
- Certifications: Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire.
- Skills: Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
