About the Position:

We are seeking an exceptional Patient Navigator/Case Manager to join our team at Community Health Northwest Florida. This role requires a highly skilled and compassionate individual who can build trust with patients, connect them with critical resources, and empower them to take control of their health.

Key Responsibilities:

  • Build Trusting Relationships: Develop strong rapport with patients, understanding their health needs and concerns.
  • Develop Personalized Care Plans: Connect patients with resources for ongoing healthcare, substance use treatment, mental health services, housing, and other community supports.
  • Care Coordination: Ensure a smooth transition from incarceration to the community by coordinating follow-up care, facilitating access to healthcare services, and assisting patients in navigating the healthcare system.
  • Empower Patients: Foster patient independence by teaching them how to navigate healthcare systems, access services, and manage their own health effectively.
  • Connect Patients with Resources: Assist patients in connecting with social services, housing programs, legal support, and other community-based resources to address barriers to care and improve overall outcomes.
  • Support Community Reintegration: Help patients reintegrate into society, ensuring they have the necessary tools and support to thrive in the community after release.

Requirements:

  • Master's Degree in Social Work or related field.
  • Experience working in healthcare, case management, or social services.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Commitment to promoting health equity and social justice.
