Key responsibilities include ensuring quality and safe delivery of services, overseeing staff assignments and staffing patterns, and providing guidance to assign appropriate staff to meet client needs.

Key Responsibilities:

• Ensures quality and safe delivery of home health care services.
• Oversees staff assignments and staffing patterns.
• Provides guidance to assign appropriate staff to a client to meet the client's needs.
• Demonstrates sound judgment and clinical knowledge in planning and decision making.
• Demonstrates competence in performing all nursing care, procedures and treatments.
• Conducts initial client assessments and reassessments and develops plans of care.
• Orients, supervises and directs field personnel in providing care.
• Coordinates with community agencies to provide for continuity of care.
• Responsible for discharge planning and the discharge process.
