Job Summary

Key Responsibilities
  • Deliver high-quality, patient-centered care through a blend of in-person and virtual visits, leveraging our proprietary technological systems to support care coordination and collaboration.
  • Collaborate with our multidisciplinary team to provide comprehensive care, including transitions of care support, social determinants of health interventions, and complex medication management.
  • Participate in care team huddles, operational huddles, and clinical partnership meetings to ensure seamless care delivery and achieve company performance goals.
  • Maintain accurate and timely clinical documentation, including medication reconciliation and patient care-related activities.
  • Engage with patients, caregivers, and clinical partners to support care coordination and promote patient engagement and experience initiatives.
Requirements
  • Nurse Practitioner or Physician Assistant with active national board certification and licensure in Tennessee.
  • Active DEA licensure and state-specific controlled substance registration, as applicable.
  • Advanced training in geriatrics and/or palliative medicine preferred.
  • Clinical experience caring for aging adults and adults with complex care needs, including home-based care and virtual care delivery.
  • Strong computer and software skills, with ability to navigate our proprietary systems.
