We are seeking a highly skilled Advanced Practice Provider to join our team at ConcertoCare. As a key member of our interdisciplinary care team, you will provide compassionate, individualized care to seniors and older adults with complex care needs in the comfort of their own homes.
Key Responsibilities- Deliver high-quality, patient-centered care through a blend of in-person and virtual visits, leveraging our proprietary technological systems to support care coordination and collaboration.
- Collaborate with our multidisciplinary team to provide comprehensive care, including transitions of care support, social determinants of health interventions, and complex medication management.
- Participate in care team huddles, operational huddles, and clinical partnership meetings to ensure seamless care delivery and achieve company performance goals.
- Maintain accurate and timely clinical documentation, including medication reconciliation and patient care-related activities.
- Engage with patients, caregivers, and clinical partners to support care coordination and promote patient engagement and experience initiatives.
- Nurse Practitioner or Physician Assistant with active national board certification and licensure in Tennessee.
- Active DEA licensure and state-specific controlled substance registration, as applicable.
- Advanced training in geriatrics and/or palliative medicine preferred.
- Clinical experience caring for aging adults and adults with complex care needs, including home-based care and virtual care delivery.
- Strong computer and software skills, with ability to navigate our proprietary systems.
- Competitive salary and bonus program, with base salary range of $120,000 to $135,000.
