Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Interim HealthCare. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Key Responsibilities
  • Provide home-based care to patients with illnesses, injuries, and chronic diseases, under the supervision of a Registered Nurse (RN)
  • Work with a team of physicians, RNs, Certified Nursing Assistants (CNAs), and aides to deliver quality patient care
  • Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
  • Assist the RN in carrying out the patient's plan of care and teaching patient self-care techniques
  • Educate family members on the patient's plan of care, medication, and home safety
  • Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in the state of Colorado
  • Minimum of 1200 hours of LPN experience, ideally in home healthcare
  • BLS/CPR certification (demonstration course required)
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
  • Pass a background check and drug test (we are required to test for THC)
