The Clinical Manager will oversee all patient/client care services and personnel, ensuring quality of patient/client care in the home. The Clinical Manager will coordinate care with the interdisciplinary team, patient/client, family, and referring agency.
Key Responsibilities:- Coordinate Patient/Client Care: Coordinate and integrate care provided by Agency staff, including contractual providers.
- Ensure Effective Communication: Ensure communication with all physicians involved in patient's/client's plan of care and other healthcare practitioners.
- Integrate Physician Orders: Integrate orders from all relevant physicians involved into the plan of care and ensure the orders are approved by the responsible physician.
- Coordinate Referrals: Coordinate referrals.
- Participate in Professional Services: Participate in activities relevant to professional services including development of qualifications. Recommend resources needed to provide care. Make patient/client and personnel assignments.
- Assess Patient/Client Needs: Assure that patient/client needs are continually assessed. Assure reassessment of the patient's/client's needs is performed by the appropriate healthcare professional when there is significant health status change in the patient/client's condition, at the physician's request, and after hospital discharge.
- Develop and Implement Plan of Care: Assure the development, implementation, and updates of the individualized plan of care in partnership with the patient/client, representative (if any) and caregiver(s).
- Ensure Provision of Services: Ensure provision of services as ordered by the physician as indicated in the plan of care, including patient/client and caregiver education and patient/client, caregiver, and family counseling. Prepare clinical notes as appropriate.
- Ensure Continuity, Quality, and Safety: Assume responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
- Collaborate on Policy Development: Collaborate on development and implementation of policies and procedures which guide and support the provision of care. Collaborate with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient/client care.
- Monitor and Participate in QAPI Program: Monitor and participate in the QAPI program and assure appropriate corrective measures are performed.
- Promote Compliance: Promote compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance, as necessary.
- Monitor Systems and Identify Problem Areas: Monitor systems and identify problem areas to Administrator both verbally and through written reports.
- Meet Continuing Education Requirements: Meet mandatory continuing education requirements of the Agency and licensing board.
- Promote Infection Control: Promote and educate regarding the concepts of the infection control and standard precautions in coordinating or performing patient/client care activities to prevent contamination and transmission of disease.
- Use Effective Interpersonal Relations: Use effective interpersonal relations and communication skills; facilitate the use of the skills by other member to achieve desirable outcomes.
- Participate in Agency Sponsored Trainings: Participate in the Agency sponsored in-service trainings.
- Ensure Consistent Care: Ensure that care offered by the Agency, including any branches, is consistently available to all patients/clients.
