About the Role:
We are seeking a skilled Physical Therapist to join our team at AgeRight Care at Home. As a Physical Therapist, you will be responsible for delivering high-quality patient care and developing personalized treatment plans to help our patients achieve their therapy goals.
Key Responsibilities:
- Conduct thorough assessments of patients to identify their physical therapy needs
- Develop and implement individualized treatment plans to address patient goals and objectives
- Provide education and training to patients and their caregivers on proper techniques and strategies for achieving optimal physical function
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination
- Monitor patient progress and adjust treatment plans as needed
Requirements:
- Current licensure as a Physical Therapist in the state of Oregon
- Graduate from an accredited Physical Therapy program
- CPR certification
- National Provider Identifier (NPI) required upon hire
Preferred Qualifications:
- Experience in home health or similar settings
- Strong organizational and communication skills
- Ability to work independently and as part of a team
