Job Description

At CREOKS Health Services, we are seeking a skilled Case Manager to join our team of dedicated professionals. As a Case Manager, you will play a vital role in providing advocacy, linkage, and referral services to individuals and families in need.

Responsibilities:

  • Provide comprehensive case management services to clients, including intake, screening, and referral.
  • Develop and implement individualized treatment plans to meet the unique needs of each client.
  • Collaborate with community agencies and healthcare providers to ensure seamless care coordination.
  • Maintain accurate and up-to-date clinical records, adhering to all relevant regulations and standards.
  • Participate in regular treatment team meetings to discuss client progress and develop strategies for improvement.
  • Stay current with all CREOKS policies and procedures, as well as relevant laws and regulations.
  • Meet productivity standards for direct service to clients, as set by CREOKS.
  • Coordinate and manage indigent medication/patient assistance programs, as needed.
  • Ensure timely and accurate billing and documentation, in accordance with CREOKS policy.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
  • Provide Psychiatric Social Rehabilitation Services to clients, as assigned by Supervisor.
  • Attend and present cases in treatment team staffing conferences, as required.
  • Maintain a full-time caseload, as assigned by the Site Director, and meet daily productivity standards.
  • Complete daily billing and clinical documentation, preferably by the start of the next workday.
  • Engage in necessary interactions with client families and other agencies to ensure effective therapeutic treatment.
  • Participate in mandatory trainings, such as CAPE and CPR, as required.
  • Implement rehabilitative curriculum and develop support materials based on best practices and evidence-based treatment models.
  • Teach rehabilitative skills to clients, focusing on activities of daily living and successful integration into community life.
  • Treat clients with varying levels of need, based on staff competency.
  • Perform all other duties as assigned.

Qualifications:

  • Minimum of a High School diploma or equivalent, with 36 months of behavioral health experience, or a Bachelor's degree in a mental health-related field, with at least 6 months of supervised experience in a treatment setting.
  • Maintain current case management certification.
  • Basic understanding of social and cultural factors influencing human behavior.
  • Knowledge of various treatment modalities.
  • Ability to work collaboratively as part of a team.
  • Core competencies in relation to individuals with co-occurring disorders.
  • Case Management certification.
