Optum Home-Based Medical Care Opportunity
We are seeking a skilled Nurse Practitioner to join our team in Denver, CO. As a member of our Home-Based Medical Care team, you will be part of a clinician-led care organization dedicated to creating a seamless health journey for patients across the care continuum.
About the Role
The successful candidate will provide medical care and management to members served by the program, participating in gathering clinical information for medical necessity determinations for service requests. You will work closely with the Landmark Health Medical Director, Director of Health Services, Corporate Director of Health Services, and UM staff to develop and implement medical management policy, clinical protocols, utilization management guidelines, and quality management programs.
Responsibilities
- Provide comprehensive assessments on newly enrolled members and bi-annually thereafter, developing a member management care plan upon enrollment and updating it as needed.
- Collaborate with UM staff and PCPs/facility staff to enable medical necessity determinations for requested medical services.
- Educate members and/or their legal representatives in disease processes affecting members and ways to manage them effectively.
- Participate in management meetings, including weekly case conference reviews, monthly staff meetings, and quarterly Quality Improvement Committee meetings.
Requirements
- Completion of Nurse Practitioner accredited program.
- Current state RN license.
- Current NP license.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
Salary and Benefits
The salary range for this role is $88,000 to $173,200 annually. UnitedHealth Group offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
