Care Manager Job Description

Connecticut Community Care is seeking a skilled Care Manager to join our team in the North Central/Eastern region. As a Care Manager, you will play a key role in coordinating the efforts of formal and informal caregivers on behalf of clients, ensuring that they receive the best possible care and support.

Key Responsibilities:
  • Conduct comprehensive, systematic assessments of clients to identify their needs and develop personalized care plans.
  • Educate clients and their families on the components of our program, service options, and DSS guidelines, including eligibility, costs, and benefits.
  • Promptly complete all client documentation, applications, and forms as required.
  • Monitor and review the continued cost-effectiveness, quality, and appropriateness of care plans and service delivery.
  • Work effectively as part of an interdisciplinary team and in conjunction with other internal and external resources and committees.
Requirements:
  • Bachelor's degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or a related field.
  • Minimum of two years' experience in healthcare or human services, with a Bachelor's degree in Nursing, Health, Social Work, Gerontology, or a related field may be substituted for one year of experience.
  • Knowledge and understanding of psychological, human development, social, health, and economic factors influencing the attitudes and behavior of individuals and families.
  • Familiarity with funding and financial sources, including Medicaid and Medicare.
