Physical Therapist Home Health Per Diem

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.
Key Responsibilities- Plan and administer physical therapy treatment and training for patients to achieve maximum physical function.
- Test and screen patients to evaluate their level of function and develop treatment plans.
- Develop and implement conditioning and rehabilitation programs to help patients achieve their goals.
- Collaborate with physicians and clinical teams to develop and revise treatment plans.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides.
- Master's degree from an accredited Physical Therapy Program.
- Minimum of one year of physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
