About the Role

HealthPro Heritage, LLC is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes. As a Physical Therapist with our company, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Evaluate and treat patients in their homes, under the orders of a physician, to restore loss of muscle strength, coordination, and body alignment, relieve pain, and improve movement and function.
  • Develop and implement comprehensive plans of care for patients, taking into account their unique needs and goals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical abilities and limitations, and develop targeted treatment plans to address these needs.
  • Provide education and training to patients and their families on proper techniques and exercises to promote optimal recovery and function.
  • Document patient progress and outcomes in a timely and accurate manner, ensuring compliance with regulatory requirements.
Requirements
  • Bachelor's or Master's degree in Physical Therapy from an accredited program.
  • Current licensure in the state of practice.
  • Prefer experience in a home health setting or similar environment.
  • Certification in areas of specialization, such as gerontology or pediatrics, is a plus.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Demonstrated ability to assess patient needs and develop targeted treatment plans.
  • Knowledge of accreditation standards and compliance requirements.
  • Ability to lift up to 50 pounds independently, and to push, pull, and transfer patients as needed.
