Responsibilities- Conduct comprehensive assessments of patients to identify their individual needs and develop personalized treatment plans.
- Provide evidence-based physical therapy interventions to improve patients' strength, balance, coordination, and range of motion.
- Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.

- Maintain accurate and timely documentation of patient care and treatment plans.
- Doctor of Physical Therapy (DPT) degree from an accredited institution.
- Current licensure as a Physical Therapist in the state of Wisconsin.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
