Clinical Pharmacist

Job Summary:

Regions Hospital seeks a skilled Clinical Pharmacist to provide professional pharmaceutical services, including verifying orders, dispensing medications, and monitoring patient medication therapies. The ideal candidate will have a strong background in pharmacy practice, excellent communication skills, and the ability to work independently with minimal direct supervision.

Key Responsibilities:
  • Interpret and verify medication orders, clarifying with providers as needed.
  • Prepare and dispense patient-specific and bulk-compounded medications, including non-sterile, sterile, chemotherapy, and parenteral nutrition preparations.
  • Issue controlled substances to patient care areas or directly to patients, maintaining records as required by law.
  • Respond to reordered meds and electronic health record in-basket requests.
  • Counsel patients about medication usage when applicable.
  • Verify technician-replenished medications and ensure safe, appropriate, cost-effective medication therapies for patients.
  • Review patient profiles, identifying potential medication errors and ensuring non-formulary medication processes are followed.
  • Participate in program development, implementation, and maintenance, collaborating with interdisciplinary teams to provide specialized medication services.
Requirements:
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or Doctor of Pharmacy degree.
  • Currently licensed to practice pharmacy in the state of Minnesota.
  • Ability to work independently with minimal direct supervision, meet and maintain departmental productivity standards, and communicate effectively (oral & written).
Preferred Qualifications:
  • One year of experience in a central inpatient role, including sterile product preparation.
  • Post-graduate training and/or BPS Board Certification in area of specialty.
