Position Overview: The Primary Care Provider plays a crucial role in delivering exceptional healthcare services to patients through direct interaction in a clinical setting as well as through various forms of mediated communication, including phone consultations and virtual visits.
Collaboration: The Primary Care Provider will work closely with Regional Directors, Clinic Directors, and Supervising Physicians to ensure optimal support and care delivery. This role involves significant interaction with the entire clinic team, including telehealth professionals and other healthcare providers, to foster a collaborative environment aimed at enhancing patient outcomes.
Responsibilities:
- Conduct comprehensive physical examinations and assessments for a diverse patient demographic, including children aged 3 and older.
- Manage chronic health conditions such as hypertension, diabetes, and asthma through effective care strategies.
- Facilitate annual wellness evaluations for both adults and children, incorporating necessary vaccinations and preventive health screenings.
- Order and interpret diagnostic tests to accurately identify and address patients' clinical issues and healthcare requirements.
- Engage with physicians and other healthcare professionals to formulate detailed patient care plans.
- Prescribe medications, suggest treatment options, and provide clear discharge instructions for patients.
- Refer patients to specialists or additional healthcare resources as necessary.
- Educate patients and their families on health maintenance and preventive practices.
- Encourage patient involvement, enhance health literacy, and support shared decision-making processes.
- Participate in initiatives aimed at quality improvement and value-based care.
- Focus on patient-centered care, ensuring continuity and fostering strong relationships between patients and providers.
