Job Summary
We are seeking a highly motivated and compassionate Hospice Registered Nurse/RN Case Manager to join our dynamic team. In this role, you will be responsible for managing a caseload of patients and providing direct nursing care services to meet the unique needs of each individual. You will work closely with other healthcare professionals to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of patients and their families.

Requirements
To be successful in this role, you must have a strong foundation in nursing practice and a passion for delivering high-quality patient-centered care. We require:
* A valid RN license in the state of employment (or eligibility to obtain)
* 1+ years of experience in hospice, home health, ICU, geriatric, or oncology settings
* Excellent communication and interpersonal skills
* Ability to work independently and as part of an interdisciplinary team
* Compassion, empathy, and a commitment to delivering exceptional patient care
