This position provides comprehensive nutrition services and education to dialysis patients and authorized caregivers in a clinical setting, focusing on maximizing nutritional well-being in a chronic-care facility for end-stage renal disease patients. The successful candidate will work in accordance with LifeBridge Health policies, procedures, and guidelines, as well as OSHA, CMS, federal, state, and local regulations.
Key Responsibilities- Assess nutritional status to determine appropriate nutrition intervention and recommendations.
- Perform initial assessments within 30 days or 13 treatments, whichever is later, or admission or transfer to the facility.
- Conduct re-assessments within 3 months of initial assessment, annually for stable patients, and monthly for unstable patients.
- Participate in interdisciplinary care plans based on assessments or re-assessments within 15 days after completion of assessment, annually for stable patients, and monthly for unstable patients.
- Review medical and diet history, consult with patients and multidisciplinary teams, document appropriate information, and nutrition progress in medical records at least monthly.
- Develop individualized medical nutrition therapy, including meal plans, for each patient.
- Provide nutrition education and counseling based on individualized patient needs, including nutrient requirements, knowledge, and abilities, medical, economic, and social situations.
- Monitor nutritional status, laboratory values, dialysis adequacy, adherence to diet, and response to nutrition-related medications and therapy to determine appropriate nutrition intervention and recommendations.
- Revise and update nutritional recommendations and care plans as needed.
- Review monthly lab results with patients and/or caregivers and provide patients with printed patient nutrition reports.
- Function as a Mineral and Bone Disorder Specialist.
- Participate in anemia management.
- Participate in rounds, patient care conferences, and continuous quality improvement teams (QAPI).
- Bachelor's degree in nutrition, dietetics, or a similar area.
- Registered Dietitian status with the Commission on Dietetic Registration.
- State licensure required in the state of Maryland or where the facility is located.
- Minimum of two years' professional work experience in clinical nutrition as a Registered Dietitian.
- Five years of experience working in a dialysis setting is preferred.
- Healthcare experience in free-standing or hospital-based dialysis units.
