Responsibilities:
  • Collaborate with patients, caregivers, and healthcare providers to ensure coordinated care and effective communication.
  • Assess readmission risk and identify barriers to care, developing strategies to address these challenges.
  • Facilitate smooth transitions from acute care to post-acute care, ensuring timely and effective communication with patients, families, and healthcare providers.
  • Promote a collaborative process and communication between healthcare team members, patients, and families to ensure integrated care services.
  • Act as a patient advocate, utilizing knowledge of applicable laws, regulations, and practice guidelines to ensure high-quality care.
Requirements:
  • Master's Degree in Social Work (MSW) required.
  • 1 year of experience as a Licensed Social Worker (LSW) or successful completion of a health-related field placement in a Master's level Social Work program required.
  • Current licensure as a Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
Work Environment:

This is a regular full-time position with a standard 8:00-4:30pm schedule. The ideal candidate will be a team player with excellent communication and interpersonal skills, able to work effectively in a fast-paced environment.
