About This Role

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Admission RN. This is a full-time position, working 1:00pm-9:30pm, with a rotating weekend commitment every 4th week.

Key Responsibilities
  • Represent HopeHealth in assigned settings and serve as a subject matter expert in the hospice benefit.
  • Present hospice services to eligible patients and caregivers, highlighting the benefits of end-of-life care.
  • Perform initial assessments of patients and families to determine the impact of terminal diagnoses on their physical, functional, psychosocial, and environmental needs.
  • Complete hospice admissions in compliance with Medicare/Medicaid and state regulations.
  • Develop individualized plans of care, including documentation of Local Coverage Determinations.
  • Participate in weekly hospice interdisciplinary group meetings to review patients on service as needed.
  • Document patient information in the electronic medical record and meet department expectations for timely completion of documentation.
  • Ensure accurate and up-to-date patient information regarding payer sources and work with referral sources to obtain necessary insurance information.
Requirements
  • Strong desire to provide patient-centered care and support.
  • RN required; BSN preferred.
  • Licensed as a Registered Nurse in Rhode Island and Massachusetts.
  • Minimum of 1-3 years of clinical experience.
  • Excellent nursing assessment, clinical, and communication skills.
