Licensed Practical Nurse

We are seeking a highly skilled Licensed Practical Nurse to join our team at MHMR Tarrant. As a Licensed Practical Nurse, you will provide high-quality nursing care to individuals in behavioral health services, working under the direction of a clinical supervisor. Your primary responsibility will be to provide focused assessment and nursing interventions to individuals assigned to your care.
Key Responsibilities- Provide quality client care using the nursing process, including focused assessment and nursing interventions.
- Collaborate with other healthcare providers to ensure adequate medical care for individuals served.
- Document all contacts related to individuals served in the program, including care coordination and medication management.
- Participate in the development and implementation of a plan of care to address individual needs.
- Provide health education and teaching to individuals, as well as document their response to information.
- Graduation from an accredited Licensed Vocational Nursing program.
- Current licensure as a Licensed Practical Nurse in the state of Texas.
- Ability to work cooperatively with multidisciplinary staff.
- Familiarity with chronic mental health, integrated health, and chronic disease populations.
- Ability to distinguish between medical and nursing interventions.
This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director. All non-exempt (hourly) employees are expected to clock in and out for each work shift, with no work done off the clock. Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.
