  • Oversee required physical therapy or rehabilitation exercises, promoting patient recovery and independence.
  • Contact emergency services when patients require hospitalization, ensuring timely and effective care.
  • Possess knowledge of Home Care reimbursement processes, ensuring compliance and optimal patient outcomes.
  • Possess knowledge of OASIS, utilizing this framework to guide patient care and assessment.
  • Have reliable transportation for home visits, ensuring timely and efficient care delivery.
  • Be able to walk up/down six flights of stairs as needed, accommodating patient needs and preferences.

Requirements:

  • Minimum of one year hospital Med Surg experience, demonstrating expertise in acute care settings.
  • One year of Home Health experience or Certified Home Care (CHHA) experience, showcasing knowledge and skills in home-based care.
  • NYS RN License & Registration in good standing, ensuring compliance with regulatory requirements.
  • Bachelor's degree, promoting a strong foundation in nursing education and practice.
  • Infection Control certificate, demonstrating commitment to patient safety and infection prevention.
  • Valid Driver License, Registration, and Automobile Insurance, ensuring safe and reliable transportation.
  • Knowledge of a second language a plus, enhancing communication and care coordination with diverse patient populations.
