Transformative Healthcare Opportunity

QuadMed is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Hartford, WI Health & Wellness Center team, serving Quad employees and their families.

Key Responsibilities:
  • Provide high-quality, patient-centered care in an outpatient setting with a focus on preventative medicine.
  • Collaborate with a multidisciplinary team to deliver comprehensive care and promote health and wellness.
  • Develop and implement individualized care plans to empower patients to achieve their full potential.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Master's Degree level Nurse Practitioner or Physician Assistant program from an accredited college/university.
  • Active and unrestricted applicable licensure and current National Board Certification.
  • Current CPR (BLS) certification or must be obtained within 60 days of hire.
  • 3+ years of experience in primary care strongly preferred.
  • NPI & DEA.
  • Background in working with pediatric patients.
  • Bilingual Spanish highly preferred.
