Job Summary

We are seeking an experienced Registered Nurse to join our PACE Clinic team as a Home Care RN. As a Home Care RN, you will work closely with our interdisciplinary team to deliver high-quality patient care in the comfort of participants' homes.

Responsibilities
  • Perform provider-ordered nursing visits to participants according to PACE visit guidelines and standards.
  • Conduct initial, comprehensive, and change-in-status participant assessments according to PACE assessment guidelines and standards.
  • Evaluate participants' individual plan of care needs and ensure quality and safe delivery of services.
  • Use medical equipment and supplies safely, effectively, and efficiently.
Required Skills and Qualifications
  • Valid license as a Registered Nurse in the state where providing care.
  • One year of experience caring for similar patient populations as assigned.
  • Basic knowledge of current, frequently used medications, including modes of administration, actions, and side effects.
  • Competency in various nursing skills, such as NPWT, complex wound care, IV therapy, catheter care, ostomy care, chronic disease management assessment, and teaching.
