We are seeking a dedicated and experienced Physical Therapist Assistant (PTA) to join our Home Health team in New Lenox, IL, for a 10-week contract. This role is perfect for a skilled PTA looking to make a significant impact in the lives of patients by providing high-quality, compassionate care in their homes.
Key Responsibilities:- Provide physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist.
- Assist in developing and implementing individualized treatment plans aimed at improving mobility, relieving pain, and preventing or limiting physical disabilities.
- Monitor patient progress and document treatments, responses, and outcomes.
- Educate patients and their families on continued physical therapy at home to promote overall wellness and recovery.
- Collaborate with the healthcare team to ensure seamless care coordination and optimal patient outcomes.
- Minimum of 1 year of Home Health PTA experience - REQUIRED
- Must hold a valid Physical Therapist Assistant license in the state of Illinois.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) - REQUIRED
- COVID-19 vaccination - REQUIRED
- Valid driver's license and car insurance - REQUIRED
