Patient Care Manager

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.
Salary: $110,000 per year, plus benefits package, mileage reimbursement, and professional growth opportunities.
Job DescriptionOverview: Our Senior Patient Care Manager will be responsible for administering the clinical aspects of our hospice program, ensuring the provision of quality care to patients, and managing the performance of assigned team members. This role requires strong leadership skills, excellent communication abilities, and a deep understanding of hospice care principles.
Key Responsibilities:
- Oversee the day-to-day operations of assigned hospice sites
- Administer the clinical aspects of the hospice program
- Ensure the provision of quality care to patients
- Manage the performance of assigned team members
- Supervise, manage performance, and direct the work of specified team members
- Assign admit and evaluation visits, schedule visits, and ensure staff are completing visits in a timely manner
- Manage the site in the absence of the Executive Director or Administrator
- Manage and oversee clinical and administrative functions
Education/Experience: Degree in nursing with an ASN or BSN from an accredited school of nursing, one-year experience as an RN (Licensed in the state of requested employment), three years' experience in direct patient care, and one year management experience.
Licenses/Certifications: Current license to practice nursing in the state of requested employment, CPR certification, current automobile insurance, and valid driver's license.
Personal Traits: Sensitivity to feelings of the terminally ill and their families before and after patient's death, flexibility, ability to demonstrate tact in dealing with co-workers and all members of the hospice team.
