YesCare is seeking a highly skilled and compassionate Licensed Behavioral Health Therapist to join our team. As a Behavioral Health Professional, you will play a critical role in providing high-quality care to our patients.
Key Responsibilities- Conduct individual and group therapeutic interventions to support patients' mental health and well-being.
- Perform psychosocial assessments and mental health intake evaluations to identify patients' needs and develop effective treatment plans.
- Provide behavioral health consultation to members of the multi-disciplinary clinical team and to correctional personnel as directed.
- Perform crisis intervention and de-escalation as needed to ensure patients' safety and well-being.
- Support and direct care coordination efforts for patients returning to the community requiring ongoing behavioral health services.
- Initiate, complete, and maintain patient treatment plans to ensure continuity of care.
- Participate in team meetings to discuss patients' progress and develop effective treatment strategies.
- Assist in planning and implementing the goals and objectives of programs and projects.
- Participate in and conduct in-service training programs to enhance staff knowledge and skills.
- Provide the necessary preparation of documentation, records, and reports to ensure compliance with regulatory requirements.
