Position Overview

As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in patient care for eye and vision services, as well as general health and well-being.

Key Responsibilities

• Deliver high-quality eye and vision care services to patients
• Manage patient scheduling, exam fees, and practice operations
• Establish your days of practice, hours of operation, and fee schedule
• Pursue opportunities to enhance patient care through equipment and support services (if state-approved)

Requirements

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate and Professional Liability Insurance Certificate
• W-9 form
