BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Georges Mills, New Hampshire.
About the Role:- As a Home Health Physical Therapist, you will provide rehabilitative care and skilled nursing services to adult and geriatric clients on a per visit basis.
- You will be an integral member of a multi-disciplinary health care team, working closely with nurses and other healthcare professionals to develop individualized care plans.
- Perform diagnostic tests and measurements, including mobility assessments, transfer status evaluations, and balance testing.
- Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, and other interventions as needed.
- Continually assess and revise care plans, collaborating with the multidisciplinary care team as necessary.
- Educate clients, family members, or caregivers on rehabilitative care and activities promoting independent living.
- Accurately document observations, interventions, and evaluations using state-of-the-art touch pad tablets.
- A current Physical Therapist license in Vermont.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
- Competitive salary: $65,000 - $80,000 per year.
