Job Title: Speech Therapist

We are seeking a skilled Speech Therapist to join our team at Astir Home Health Care in Bloomfield, NJ.

Job Summary:

The Speech Therapist will provide skilled speech therapy services to patients in their homes, as per the physician's orders. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong passion for helping patients achieve their goals.

Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients' needs, including OASIS assessments.
  • Evaluate patients' significant others and home situations to determine required instruction and assistance.
  • Develop and implement interdisciplinary patient care plans, including speech therapy, in consultation with physicians.
  • Guide and instruct patients in prescribed therapeutic self-care and creative activities to improve independence and functioning.
  • Teach, supervise, and counsel patients and their families in total speech therapy programs and related problems.
  • Establish goals based on patients' current levels of functioning and potential for improvement, and perform re-evaluations as indicated.
  • Makes arrangements for outpatient services when procedures cannot be given in the home.
  • Evaluate the effectiveness of speech therapy intervention and update patient care plans as needed.
  • Prepare clinical/progress notes on the day of the visit, which are incorporated into the chart within 48 hours.
  • Communicate with physicians regularly regarding patients' status, obtaining additional orders as necessary.
  • Communicate with other team members and instruct them in speech therapy techniques when appropriate.
  • Supervise Home Health Aides and Speech Therapy Assistants as per agency policy.
  • Participate in agency activities, in-services, and meetings as appropriate.
  • Confirm weekly scheduling of visits with Clinical Managers to coordinate agency visits.
  • Participate in discharge activities and complete speech therapy discharge summaries.
Requirements:
  • Graduate of an accredited Speech Therapy school.
  • Eligible to take or has successfully completed the entry-level certification examination for Speech Therapists developed and administered by the National Board for Certification in Speech Therapy, Inc.
  • Licensed or registered by the state of New Jersey/New York.
  • At least one year of institutional experience.
  • Criminal background check.
  • Current CPR certification.
Working Conditions:

The ability to drive and access patients' homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of patients' conditions and perform patient care. Must be able to effectively communicate in English.

Equipment Operation:

Speech therapy equipment and other medical equipment will be utilized. Hand washing materials.
