About the Role

We are looking for a compassionate and dedicated Physical Therapist to work with our busy and expanding Home Health office. This is a Monday-to-Friday position, with the opportunity to work in a dynamic and supportive environment. You will be responsible for conducting one-on-one visits, assessing patient needs, and developing personalized treatment plans to improve their functional status and overall well-being.

Responsibilities
  • Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
