About the Role

VNA Health Group is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes in Essex County.

The ideal candidate will have a strong background in nursing and excellent communication skills to work effectively with patients, families, and healthcare teams.

  • Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
  • Provide comprehensive nursing care, including medication management, wound care, and education on disease management and self-care.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress, updates, and outcomes in a timely and accurate manner.
  • Identify teaching needs and develop strategies to educate patients and families on health management and disease prevention.
Requirements
  • RN with a Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a Registered Nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and proficiency in electronic medical records.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
