SUMMARY OF
POSITION:
The Treatment Court Clinical Case Manager is tasked with delivering essential support to individuals engaged with the Washington County Treatment Court, specifically those identified for case management services related to substance use disorders.
This role involves collaboration with the Treatment Court Team, serving as a vital link between the agency and Treatment Court administration, while facilitating connections to community resources that are crucial for achieving favorable treatment outcomes.
KEY RESPONSIBILITIES:
Providing comprehensive support services to clients, which encompasses but is not limited to:
1. Outreach visits,
2. Supportive counseling,
3. Crisis intervention,
4. Assistance with independent living skills,
5. Transportation of clients in a personal vehicle when clinically necessary.
Assisting clients in securing safe and suitable housing, as well as aiding in their employment search.
Empowering clients to take charge of their lives by collaborating with them to develop personalized care plans, equipping them with the necessary skills for community living, and providing psychoeducation as needed.
Conducting ongoing assessments and treatment planning, including coordinating treatment team meetings as required.
Participating in mandatory meetings and training sessions, including individual and/or group supervision and other agency committee responsibilities.
Completing all required documentation punctually, including agency records and generating reports as needed for grant management.

QUALIFICATIONS:
A bachelor's degree in human services or criminal justice is required; a master's degree is preferred.
A minimum of 2 years of experience in human services, with a focus on substance use preferred.
AAP certification or a commitment to obtain AAP certification within 180 days of hire is required.
Ability to transition from a seated to a standing position with or without accommodation.
Capability to transport clients in a personal vehicle to community-based events without accommodation.
Access to a safe, reliable, and insured vehicle.
