Key Responsibilities
  1. Support and Empowerment: Provide guidance and support to Direct Support Professionals in establishing a positive and empowering atmosphere in the home and community. Model best practices for supporting individuals with their daily needs and goals.
  2. Communication and Collaboration: Facilitate open and effective communication among individuals, staff, and community resources. Ensure that all parties are informed and involved in the individual's care and support plan.
  3. Quality of Life: Assess and address the individual's quality of life, including their physical, emotional, and social well-being. Work with the individual and their support team to identify and implement strategies to improve their quality of life.
  4. Documentation and Record-Keeping: Maintain accurate and up-to-date documentation of the individual's care and support plan, including medication logs, incident reports, and household finances.
  5. Medication Management: Ensure that all medications are administered and documented correctly, and that any concerns or issues are addressed promptly.
  6. Daily Life Skills: Assist individuals with daily life skills, such as personal care, meal preparation, and household management.
  7. Food and Household Management: Ensure that adequate food and household supplies are available, safely handled, and stored. Provide nutritious meals and snacks, and address any concerns regarding the individual's diet and exercise.
  8. Transportation and Appointments: Assist individuals with transportation to appointments, activities, and visitations, and provide guidance and support to Direct Support Professionals in this area.
  9. Incident Response: Recognize and respond to concerns of neglect and/or abuse, and communicate all major incidents to the Supports Directors and Program Specialist.
  10. Community Engagement: Support individuals in areas such as socialization, community resources, vocational development, and interpersonal skills.
  11. Staff Supervision and Training: Supervise and guide Direct Support Professionals in their roles, provide training and support as needed, and ensure that they are equipped to provide high-quality care and support.
  12. Communication and Collaboration: Maintain open lines of communication with the team, ensure continuity of care, and consistency of home operations and individual program plans.
  13. Interface with Care Management: Interface with care management, family, and community resource systems as necessary, and consult with the Program Services Executive and/or Program Specialists as needed.
  14. Availability and Flexibility: Be available to answer phone calls and emails after work hours if needed, and provide coverage in the homes if required due to staff shortages or emergencies.
