Key Responsibilities- Support and Empowerment: Provide guidance and support to Direct Support Professionals in establishing a positive and empowering atmosphere in the home and community. Model best practices for supporting individuals with their daily needs and goals.
- Communication and Collaboration: Facilitate open and effective communication among individuals, staff, and community resources. Ensure that all parties are informed and involved in the individual's care and support plan.
- Quality of Life: Assess and address the individual's quality of life, including their physical, emotional, and social well-being. Work with the individual and their support team to identify and implement strategies to improve their quality of life.
- Documentation and Record-Keeping: Maintain accurate and up-to-date documentation of the individual's care and support plan, including medication logs, incident reports, and household finances.
- Medication Management: Ensure that all medications are administered and documented correctly, and that any concerns or issues are addressed promptly.
- Daily Life Skills: Assist individuals with daily life skills, such as personal care, meal preparation, and household management.
- Food and Household Management: Ensure that adequate food and household supplies are available, safely handled, and stored. Provide nutritious meals and snacks, and address any concerns regarding the individual's diet and exercise.
- Transportation and Appointments: Assist individuals with transportation to appointments, activities, and visitations, and provide guidance and support to Direct Support Professionals in this area.
- Incident Response: Recognize and respond to concerns of neglect and/or abuse, and communicate all major incidents to the Supports Directors and Program Specialist.
- Community Engagement: Support individuals in areas such as socialization, community resources, vocational development, and interpersonal skills.
- Staff Supervision and Training: Supervise and guide Direct Support Professionals in their roles, provide training and support as needed, and ensure that they are equipped to provide high-quality care and support.
- Communication and Collaboration: Maintain open lines of communication with the team, ensure continuity of care, and consistency of home operations and individual program plans.
- Interface with Care Management: Interface with care management, family, and community resource systems as necessary, and consult with the Program Services Executive and/or Program Specialists as needed.
- Availability and Flexibility: Be available to answer phone calls and emails after work hours if needed, and provide coverage in the homes if required due to staff shortages or emergencies.
