About the Job
The University of Iowa Hospitals & Clinics Department of Nursing seeks a skilled Discharge Planning Specialist to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division. This role is ideal for a motivated professional with a passion for pediatric care and a desire to make a meaningful impact in the lives of our patients and families.

Key Responsibilities
In this role, you will:
  • Work collaboratively with the COC team members, University of Iowa Stead Family Children's Hospital faculty/staff, patients and families, and external providers/agencies in coordinating the care of designated patients throughout their inpatient admission(s), discharge to home/community agencies, and return to the University of Iowa Children's Hospital for follow-up ambulatory visits.
  • Provide services coordination, consultation, and resource and referral assistance; serve as a liaison between the faculty and staff of the University of Iowa Stead Family Children's Hospital inpatient units and various medical/surgical subspecialty clinics, the regional Child Health Specialty clinics, and numerous interdisciplinary community providers/agencies.
