Job Summary: We are seeking a skilled and compassionate Home Health Nurse to join our team at Providence Medford Medical Center. As a Home Health Nurse, you will have the opportunity to make a meaningful difference in the lives of patients and their families by providing high-quality, patient-centered care in the comfort of their own homes.
About the Role: Our ideal candidate will be a licensed practical nurse (LPN) with experience in home health, hospice or public health. You will work alongside an interdisciplinary team to provide whole-person quality care to patients in need, utilizing your specialized training to deliver world-class health with human connection.

Required Qualifications: To be successful in this role, you must be a graduate of an accredited LPN/LVN school of nursing, possess National Provider BLS - American Heart Association certification within 30 days of hire, hold an Oregon Licensed Practical Nurse license upon hire, and demonstrate excellent communication and interpersonal skills.
