Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Trillium Health NY. As a key member of our primary care team, you will collaborate with healthcare providers and patients to deliver high-quality, patient-centered care.

• Provide direct patient care and chronic disease management through collaborative drug therapy management (CDTM)
• Collaborate with pharmacy team and provider team to ensure access to medications and improve healthcare resource stewardship
• Participate in clinical pharmacy initiatives and quality improvement projects to enhance patient care and outcomes
• Provide education and training to patients, providers, and medical staff on chronic disease states and medication management
• Supervise and precept pharmacy support personnel and students

• Doctor of Pharmacy or BS Pharmacy degree
• Completion of PGY 1 residency or 3 years clinical experience
• Current NYS pharmacist license with immunizer credential
• Current NYS Collaborative Drug Therapy Management credential, or ability to obtain within 3 months of hire
• Minimum of 1 year experience in primary care pharmacy involving direct, collaborative patient care
• Experience precepting students and/or residents

• Current Board of Pharmacy Specialties credential
• Current American Academy of HIV Medicine Pharmacist (AAHIVP) credential
• Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM)
• Completion of PGY 2 residency in primary care, ambulatory care or similar experience
• Experience working in community-based healthcare setting

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment. A valid NYS driver's license and reliable vehicle are required for this position.
