Job Overview

LOCATION:
Kissel Hill Outpatient Center

HOURS:
Full-time (36 hours per week). Three 12-hour shifts. Rotating weekends and holidays. Office Hours - Monday to Friday 9am-9pm. Saturday and Sunday 8am-8pm.

SIGN ON BONUS:
Up to $2,500. Prorated based on FTE, with a 2-year commitment. External applicants only.

POSITION SUMMARY:
The Clinical Practice Nurse is responsible for delivering nursing services to patients within a healthcare setting. The role encompasses care for patients across various age groups, including:
- Pediatric (infant to 12 years)
- Adolescent (13 to 17 years)
- Adult (18 to 64 years)
- Geriatric (over 64 years)

KEY RESPONSIBILITIES:
Qualified candidates must demonstrate the ability to perform the following duties effectively:
  • Provide clinical guidance via phone or electronic communication, adhering to regulatory standards and protocols.
  • Consult with healthcare providers as necessary.
  • Deliver patient-specific education on preventive care, chronic disease management, and medication.
  • Prepare patients for consultations using established rooming standards.
  • Administer injections and medications as prescribed, following departmental protocols.
  • Assist providers with various procedures and examinations.
  • Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
  • Collect and prepare laboratory specimens for testing.
  • Authorize prescription refills as directed by the provider.
  • Assist in tracking tests and monitoring overdue results.
  • Perform wound care, including suture removal and dressing changes.
  • Ensure exam rooms are well-stocked and sanitized after each patient visit.
  • Maintain and clean medical instruments and equipment post-procedure.
  • Support providers and patients in reviewing and updating care plans, assessing patient progress.
  • Engage in proactive outreach for patients needing preventive and chronic disease services.
  • Coordinate care among specialists and facilitate transitions of care.
  • Participate in performance improvement initiatives to enhance patient experience and care quality.
  • Engage in daily huddles with the provider.
  • Act as a patient experience advocate, fostering positive interactions with patients, visitors, and colleagues.
  • Maintain confidentiality of patient information in accordance with health privacy policies.

ADDITIONAL FUNCTIONS:
The following duties are considered secondary to the primary responsibilities:
  • Perform various front office tasks as needed.
  • Participate in on-call responsibilities as required.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS:
  • Completion of a LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
  • Ability to travel to multiple sites as required.

PREFERRED QUALIFICATIONS:
  • 1-2 years of nursing experience in a primary or specialty care setting.
  • Prior experience in triage, emergency room, cardiac care, or intensive care.
