Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Quality Talent Group. As a key member of our healthcare team, you will provide high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide and document skilled nursing care to patients in accordance with their individual care plans and physician orders.
  • Conduct regular assessments of patient condition and care needs, and report changes to the supervisor and/or physician as necessary.
  • Develop and implement individualized care plans to achieve optimal patient outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
Work Environment
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
